INTRODUCTION The anal canal carcinomas (ACC) represent 4% from the gastro-intestinal carcinomas (Clark et al, 2004) The ACC represented 2,37% from 507 colorectal carcinomas in our cases

MATERIAL AND METHODS For statistical analysis were studied 507 colorectal carcinomas (CRC) For immunohistochemistry, 20 cases with CRC: - 5 well-differentiated - 3 intermediary differentiated - 4 poorly differentiated - 6 mucinous carcinomas - 2 malignant polyps 12 cases with ACC: - 3 well-differentiated - 3 intermediary differentiated - 2 poorly differentiated - 2 mucinous carcinomas - 2 squamous cell carcinomas (SCC) Statistical Program Graph Pad In Stat 3 -Trial Version

Sex and age of patients The ACC were diagnosed more frequently at females (58%) than CRC (43%) - p=0,004 The ACC were diagnosed at younger ages than CRC (p<0,0001). The median age was 65 years for CRC and 57 years for ACC

ASPECTS OF ANGIOGENESIS CD31, CD105 and VEGF CD31, a platelet endothelial cell adhesion molecule, markes neoformed, immature and mature vessels CD105 markes just the proliferating endothelium, the neoformed vessels but it is not a specifical tumour endothelial marker VEGF (Vascular Endothelial Growth Factor) is produced by mastocites, tumoral cells and other cells in hypoxic conditions. Its expression is observed in the cytoplasma of tumor cells.

Aspects of angiogenesis In ACC CD31 immunostain was correlated with CD105 expression, independently by microscopic field (p<0,0001) In CRC CD105 was correlated with CD31 immunostain at 400x (p=0,0004) but not at 200x magnification (p=0,39) Only in ACC, the intensity of CD31 MVD seemed to increase with age but not statistically significant (p=0,25)

Aspects of angiogenesis In CRC we not observed a statistical difference between intratumoral (13,23±3,98%) and peritumoral CD105 MVD average area (12,50±4,43%) neither peritumoral (34,05±8,52%) and intratumoral CD31 MVD average area (29,65±9,78%). Neither in ACC we not observed a significant difference between CD31 or CD105 regarding the intra- or peritumoral area.

The correlations of angiogenesis In CRC, both CD31 and CD105 were not correlated with grade of differentiation. In CRC, only CD31 was reversly correlated with level of penetration (p=0,05), being higher in pT2 (39±5,41%) than pT3 or pT4 (30±3,70% respectively 15,5±1,23%) In CRC, CD31 was reversly correlated (p=0,05) with lymph node involvment (37,9±7,44% in pN0, 29,2±4,23% in pN1 and 11±2,52% in pN2). In ACC both CD31 and CD105 immunostains were not correlated with depth of invasion, lymph node involvement or grade of differentiation.

Only in ACC, CD31 and CD105 were correlated with size of tumor (p=0,0002) but did not correlated with pTNM staging (p=0,60)

Expression of VEGF In CRC VEGF was expressed in 80% of cases. A reverse correlation between VEGF and CD31 was observed (p=0,004). VEGF was not correlated, in CRC, with CD105 (p=0,56) or other prognostic factors. VEGF was expressed in 25% of ACC and did not correlated with CD105 (p=0,81), CD31 (p=0,08) or other prognostic factors.

DISCUSSIONS ACC appear around the age of 55, more frequently at females (Valentini et al, 1997) – 57 years, 58% in our cases Grossly, in the majority of cases, ACC are ulcerated (Dougherty et all, 1985) - 58% polypoid-ulcerated, 42% ulcero-infiltrated Many papers reveal the predominance of SCC and basaloid carcinomas in anal canal (Dem et al, 2000) - 17% in our cases In ACC, the lymph nodes are involved when diagnosis is made (Grabenbauer et al, 2005) – 58% pN0

DISCUSSIONS In ACC, no correlation between CD31 value and histologic type, lymph node involvment, patients age or neoplastic relapse (Indinnimeo et al, 2001) The antiangiogenic treatment prolonge the life in CRC with metastasis (Hurwitz et al, 2004) - we observe a higher MVD in non-metastatic CRC

DISCUSSIONS In CRC, VEGF is correlated with the depth of invasion, but it is not correlated with other classical prognostic factors or liver metastasis (Saad,2004) VEGF was correlated with CD31 in CRC (Saad,2006) – reverse correlation in our cases 55% of SCC expressed VEGF (Alvarez, 2006) – both two cases were negative in our study

CONCLUSIONS The ACC seems to have particular behaviour compared with CRC The angiogenesis in ACC are not so high like in colorectal carcinomas and the immature neoformed vessels are more frequently

CONCLUSIONS 3. The percent of VEGF positive cases with ACC is smaller compared with CRC. In ACC, the antiangiogenic treatment that regard the VEGF inhibition seems to not be so efficient as in CRC. 4. The radiotherapy could stop the angiogenesis and could inhibites the vessels` maturation.
