| The sudden and random increase in an oxygenators premembrane pressures and membrane pressure differentials during Cardiopulmonary Bypass has drawn considerable attention in the perfusion community over the past few years. Several terms have been applied to this phenomenon but the term that best describes this unexpected event is High Pressure Excursion.
The exact causes of High Pressure Excursions are still uncertain, but nonetheless widely speculated. However, one thing that is certain is their increased appearance seems to be very closely related to the removal of Human Serum Albumin from bypass priming solutions.
In an attempt to determine why High Pressure Excursions occur during cardiopulmonary bypass, we present our findings in a multicenter, retrospective analysis of 2,696 cardiopulmonary bypass cases. Of the 31 cases of High Pressure Excursions that were documented from the analysis, 60 previously suspect preoperative and perioperative variables were gathered from the participating tertiary care centers. Our findings indicate that these pressure excursions had an occurrence of 1.14 %, in the three centers involved with this analysis. The largest occurrence of High Pressure Excursions tended to be in male (87.1%) coronary artery disease patients (96.8%) during the presence of the IV anesthetic Diprivan (74.2%). Conclusions: High Pressure Excursions are not perfusate related as it occurred in the presence of three different perfusate combinations. They also do not appear to be oxygenator related or exclusive to hypothermic temperatures and heat exchangers. However, they are found to be dramatically reduced in the presence of albumin and coated circuits.
